GENERAL DESCRIPTION OF POSITION
Under the direct supervision of the Community Services Director, the Community Services Deputy Director or Program Coordinator, is responsible for case management services for families and individuals eligible for the assigned Program, other Community Services Department programs and services, and for assisting in the development of programs and services, as needed, for low-income families and individuals. Responsible for outreach, eligibility determination, records management, referrals, follow-up services, identifying and securing community resources for participants, maintaining partnerships, and representing the agency at collaborative and partner meetings.
Requirements
Bachelor’s degree in social sciences or related field or at least two years college and 2 years experience in a similar position. Ability to evaluate information to determine compliance with standards, by using relevant information and individual judgment to determine whether events or processes comply with laws, regulations, policies, procedures, or standards. Must have a valid driver's license, good driving record, and motor vehicle insurance coverage.
