We are seeking a dedicated and experienced Peer Support Qualified Professional to join our team. In this role, you will be responsible for the initial development and ongoing revision of Person-Centered Plans (PCPs) while ensuring their successful implementation. You will also coordinate and oversee the initial and ongoing assessment activities for clients.
Key Responsibilities:
- Complete the initial development of Person-Centered Plans (PCPs) for clients, ensuring they align with NC Medicaid guidelines and meet the unique needs of each individual.
- Revise and update PCPs regularly to reflect any changes in client progress, goals, or needs.
- Coordinate and oversee initial and ongoing assessments to evaluate client needs and progress.
- Ensure the successful implementation of PCPs by collaborating with Peer Support Specialists and other team members.
- Monitor and support clients in achieving their goals, ensuring that services provided are effective and person-centered.
- Maintain accurate and timely documentation of all assessments, PCP revisions, and progress notes in compliance with organizational and regulatory standards.
- Communicate regularly with clients, families, and care teams to provide updates on progress and address any concerns.
- Participate in team meetings and trainings to ensure continuity of care and to stay informed of best practices.
- Assist in resource coordination and referrals to additional services as needed to support client goals.
Qualifications:
- Bachelor’s degree in a human services field or equivalent combination of education and experience.
- Must meet the qualifications for a Peer Support Qualified Professional according to NC Medicaid guidelines.
- At least 2 years of experience working with individuals in behavioral health or peer support services.
- Strong knowledge of Person-Centered Planning and assessment protocols.
- Excellent communication, documentation, and organizational skills.
- Ability to work independently and as part of a team in on-site environments.
- Proficiency with electronic health records (EHR) systems, preferably SimplePractice or similar platforms.
