The intake coordinator will serve a vital role for CTSCD, Inc. as the organization expands its services and footprint. This position at CTSCD, Inc. will report to CTSCD, Inc.’s Operations Director and is responsible for the accurate monitoring and coordination of medical and social documentation.

Accurately track and organize records related to client intakes and referrals.- Coordinate enrollments and intake services to clients and potential clients.
- Respond to telephone inquiries from persons seeking mental health care or information about mental health-related services.
- Support Clinical Director in file auditing and ensuring a quality continuum of care.
- Run benefits verification and input clients into the electronic medical records system.
- Collaborate with case management and clinical teams to ensure the client is connected to services.
- Review all paperwork accompanying patients to ensure adherence with Medicaid and client intake policies.
Must be at least 18 years of age, be able to read, write and affect both written and verbal communication successfully.
- Possess no substantiated findings of abuse or neglect listed on IL Health Care Registry, and no criminal convictions.
- Completion of, at minimum, GED or High school diploma with population being served. one year of residential experience or demonstrates sound clinical knowledge of the service provision.
- Must have adequate transportation to provide services in the community.
- Criminal records, fingerprinting, and HCPR check is a prerequisite for employment consideration.
- Must complete all CTS Community Development training requirements.
- Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
- Must successfully complete all background checks and other onboarding requirements.
- Knowledgeable of Chicago’s Austin and Garfield Park communities in addition to the socioeconomic hardships of the area
- Great written and verbal communication skills
- Excellent customer service skills.
- Willingness to work with clients from diverse backgrounds from a decolonized trauma-informed lens.
- Ability to navigate Microsoft office software such as Teams, Word, Excel, Outlook and Power Point.
Knowledgeable of or willing to be trained in crisis intervention/ safety training
