What You Need to Apply:
Valid New Jersey Registered Nurse's License or Advanced Degree for Nursing Specialists
Two (2) years of experience working with individuals with chronic mental illness
Some supervisory experience preferred
Knowledge of community resources and agencies
Willingness to work a flexible 40-hour schedule, including some weekends and holidays as needed
Willingness to accept on-call rotation and work as part of a team
Valid New Jersey driver's license with a five-year abstract meeting agency standards
Position Overview:
As a PACT Nurse Specialist, you will serve as an essential member of our PACT Team, which provides intensive, community-based services to adults with serious mental illness. You will be responsible for conducting nursing assessments, both upon program entry and annually, and will assess mental health symptoms and behaviors in response to prescribed medications. In this role, you will closely monitor individuals for side effects and support their understanding of medication benefits and compliance.
You will oversee the monthly inventory of all medications and medical supplies, and take the lead in preparing and organizing the upcoming month's medical agenda. As part of your ongoing responsibilities, you will ensure accurate records of medication orders, track all received and distributed medications, and manage medication administration in coordination with the team. Beyond medical responsibilities, you will also provide supportive counseling and psychoeducation, empowering individuals to manage, cope with, and gain mastery over their symptoms and disability.
Additionally, you will work collaboratively with other team members during treatment planning, case reviews, and regular team meetings, helping to create and adjust comprehensive service plans that meet the evolving needs of those we serve. Your contributions will have a direct impact on the health, independence, and overall quality of life of our clients.
