We operate five group homes and a day program, ensuring individuals receive the support they need to thrive in their communities.
Position Overview
We are seeking a Qualified Professional (QP) to provide clinical oversight and support for our five residential programs. The QP is responsible for overall coordination of services and supports, develop and implement individualized service plans, train and support staff, and advocate for the well-being of individuals served. The ideal candidate is a compassionate, organized, and knowledgeable professional with experience in IDD services.
Key Responsibilities
- Oversee the development and implementation of Person-Centered Plans (PCPs) for individuals in residential programs.
- Ensure compliance with state and federal regulations, including Medicaid, NC Innovations Waiver, and DHHS requirements.
- Conduct assessments to monitor individuals’ progress, needs, and well-being.
- Provide training, coaching, and support to Direct Support Professionals (DSPs) and residential staff.
- Serve as the liaison between CREST, families, guardians, and external agencies.
- Monitor and document incident reports, behavioral interventions, and risk management plans.
- Participate in interdisciplinary team meetings to review and update service plans.
- Advocate for individuals’ rights, dignity, and community inclusion.
- Qualifications
- Bachelor’s degree in Social Work, Psychology, Special Education, or a related human services field.
- Two years of experience working with individuals with IDD.
- Knowledge of North Carolina service definitions, Medicaid requirements, and DHHS regulations.
- Strong organizational and documentation skills.
- Ability to train and support staff in best practices for IDD care.
- Excellent communication and advocacy skills.
- Proficiency in Microsoft Office and electronic health records (EHR) systems.
- Must have a valid driver’s license and reliable transportation.
