Job Title: SOAR Case Manager
Position Summary:
SSI/SSDI Outreach, Access, and Recovery (SOAR) is a model that helps individuals experiencing or at-risk for homelessness who have mental illness and/or a co-occurring substance use disorder, or other medical impairments apply for Social Security disability benefits.
Qualifications:
A Bachelor’s degree in a related field and a minimum one year of related experience. Evidence of ongoing training and education in related areas such as mental illness, substance abuse, and/or homelessness. Superior organizational skills and attention to detail. Computer experience, including troubleshooting and problem solving and proficiency in MS Office applications (Word, Excel) and database management. Excellent interpersonal and communication skills. Ability to prioritize multiple tasks and meet frequent deadlines. Experience engaging with a diverse population and ability to respond appropriately to crisis situations. Possess an interest in social justice and have experience doing advocacy work for underserved populations. Good writing skills and the ability to analyze extensive data and create written reports with accuracy and brevity. Basic knowledge of medical and psychiatric terminology and ability to write using same. Ability to maintain professional boundaries and engage a challenging population and in nontraditional work conditions. Experience with providing outreach and successful engagement to a diverse population, includes working with clients, developing trust and conducting interviews in non-traditional settings and unstructured environments. Possess a valid state driver’s license with a clean driving record.
Hours:
Monday through Friday 8:00am until 4:30pm. Other times may be required with the job or adjusted by your supervisor.
This position reports directly to the Team Leader if located in an outpatient facility and/or Program Director, if in a residential program.
Essential Duties and Responsibilities:
The SOAR Case Manager must be able to work in the community and meet homeless consumers where they are, including in the woods, under bridges, and in abandoned houses. This position requires a minimum
of a 4 year degree in human services; at least 1 year of experience working directly with individuals who have mental illness and/or are dually diagnosed; evidence of ongoing training and education in related areas such as mental illness, substance abuse, and/or homelessness; and an ability to work in non-traditional settings and unstructured environments. The SOAR Case Manager will participate in ongoing trainings offered through SAMHSA-approved SOAR Technical Assistance
Center.
Position Responsibilities:
1. Become SOAR Certified within your first 2 months through SAMHSA-approved SOAR online training course
2. Work with the referral sources and community partners to identify candidates through team meetings, outreaches, and referrals covering Cumberland Rivers 8 county region
3. Initiate paperwork with consumers as referred to the program by filing initial documentation of representation with SSA office
4. Complete interviews with consumers to gather information to complete SSI/SSDI applications
5. Gather medical records and other information to complete SSI/SSDI applications
6. Write SOAR Medical Summary Reports for consumer applications
7. Accompany consumers to appointments at the Social Security Administration
8. Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for case
9. Assist the team with administrative tasks as needed
10. Carry and maintain a minimum of a 15-person case load
Work Environment:
Position Work Conditions, include but are not limited to:
1. Frequent travel by foot, car or other means appropriate to making contact with population
2. Must be able to adjust to the environment of the target population including making visits to homeless encampments shelters and personal living environments when needed
3. Sitting for extended periods of time
4. Dexterity of hands and fingers to operate a computer keyboard, mouse, and other devices and objects
5. Physically able to participate in training sessions, presentations, and meetings
6. Travel for the purpose of meeting with clients, training and collaborating with stakeholders, or off-site personnel/management

- Hours of work are structured 7.5 – 8.0 hour blocks of time but may be subject to irregular and/or flexible hours based on needs or demands.
- Position may require duties to include routine or reasonable anticipated tasks or procedures where there is a degree of actual or potential exposure to blood or other infectious materials.
- All staff are required to participate in yearly trainings regarding infection control and safety in the environment including hazardous waste and blood borne pathogens.
- All new staff must attend training regarding handling physical, verbal threats, acts of violence or other escalating and potential dangerous situations including when police need to be summoned.
- All staff must be certified in Cardiopulmonary Resuscitation within one month of employment and maintain the certification, if providing direct care.
