Position: CCBHC Care Coordinator Location: Greencastle, Indiana

About the Role: As a CCBHC Care Coordinator, you'll provide face-to-face and virtual support to individuals and families, helping them navigate complex physical and behavioral health needs. You'll be part of our Clinical-Medical Services Team and work closely with consumers' treatment teams to promote resiliency and community participation.

Key Responsibilities:

· Assess and screen for functional and health needs.

· Manage whole person care and coordinate services.

· Connect individuals with necessary resources and support.

· Develop and update person-centered treatment plans.

· Collaborate with providers and systems for comprehensive care.

· Ensure accurate documentation and crisis planning.

· Promote health education and patient advocacy.

Qualifications:

· Bachelor's degree in human services, nursing, or related field; or

· Associate's degree with 4 years of direct care experience; or

· High school diploma/GED with 4 years of direct care experience.
