Job Overview
The Client Care Coordinator performs caregiver services for the client as necessary to maintain the personal care and comfort of the client. Client Care Coordinators are responsible for minimal supervisory duties such as as coordination and implementation of the client services when necessary.
Hours of Work- This position offers part-time, full-time, or temporary employment dependent on client and organization needs. This position has no set or recurring schedule. Client Care Coordinators are expected to be on call 24 hours a day, 7 days a week, 365 days a year, with use of designated use of company phone when available or during rotation.
Duties
- Proficiency in medical terminology to effectively communicate with healthcare professionals and patients.
- Strong organizational skills with the ability to manage multiple tasks simultaneously.
- Excellent interpersonal skills to foster positive relationships with patients and staff.
- Attention to detail to ensure accuracy in patient records and documentation.
- Ability to work collaboratively within a team-oriented environment while also being self-motivated.
- Assist with bathing, personal hygiene, grooming, oral care, client dressing and undressing.
- Assist with toileting, urinal or bedpan, incontinent care, medication reminders, transportation of clients
- Assist with client's laundry and bed linen, exercises, ambulation, and transfer activities.
- Assist with meal planning, purchase of food, meal preparation, clean up and feeding client in accordance with care plan.
- ADL documentation, reporting incident/accidents of clients and caregivers directly to Program Manager and complete incident reports
Skills
- Must be 18 years of age
- High school diploma or equivalent
- One (1) year of supervisory experience in homecare.
- Demonstrate sympathetic and positive attitude while caring for others.
- Successfully complete and pass all caregiver training. Successfully complete ongoing compliance with licensing, certification, continuing education, background checks, and drug and alcohol testing.
- Ability to listen and communicate in all assigned tasks as indicated in plan of care.
- Valid drivers license and reliable transportation with valid insurance.
- Remain flexible, resilient, calm, and maintain a sense of humor in all situations.
- Present well-groomed image that reflects the professionalism of the business.
