Job Summary
We are seeking a dedicated and compassionate Case Manager to join a growing agency. Duties
- Conduct thorough assessments of clients to identify needs and develop individualized service plans.
- Facilitate access to resources such as healthcare services, housing assistance, and workforce development programs.
- Collaborate with multidisciplinary teams to ensure comprehensive care for clients.
- Monitor client progress and adjust service plans as necessary to meet evolving needs.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with regulatory standards.
- Advocate for clients within the system to ensure they receive necessary services and support.
Experience
Candidates should possess the following qualifications:
Bachelor's degree and at least 5 years experience in social work or behavioral health is a must.
- Familiarity with Section 8 housing programs and Medicaid/Medicare is a plus.
- Proven skills in crisis management and conflict resolution.
- Strong organizational skills with the ability to manage multiple cases effectively and independently.
- Excellent communication skills for engaging with clients from diverse backgrounds.
- A commitment to continuous professional development in areas related to workforce development and addiction counseling.
