JOB SUMMARY:
This role supervises health education activities including individual and group activities within the health and senior centers. Activities include evaluating health coaching patients, providing Health Education Support for Food Pharmacy Maintenance Programs, supervise the food and grocery programs, recruiting clients for participation in group education activities, presenting group health education courses, and representing the agency in community meetings. Key team member of the Senior Vitality Program, providing health and technology programs that assist clients access health resources and information online.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Oversee daily operations of department.
- Practice and reinforce the mission, vision and values of the organization.
- Participate in the interviewing, hiring, and orienting all direct reports.
- Maintain appropriate staffing levels.
- Provide initial and ongoing training to team members.
- Assign duties and work schedules to direct reports and provide on-site guidance of established procedures.
- Review and approve timecards, including time off requests. Make appropriate edits, when needed.
- Monitor overtime, doubletime, and missed meal breaks. If abuse is apparent, meet with employee(s) to discuss. If abuse continues, work with HR for disciplinary actions.
- Provide support, direction, and supervision to team members and assist them with any questions or problems.
- Address any performance problems.
- Work with team to identify appropriate clients for health education interventions.
- Supervise the development of group presentation(s) of all health education courses including but not limited to Chronic Disease Self-Management – Stanford model; Diabetes Empowerment Education Program (DEEP); Tai Chi for Balance, and other appropriate courses and activities.
- Attend required training to continue certifications in each course.
- Project Senior Vitality: Supervise team working with clients to identify goals for health self-management and improvement in health coaching sessions, meeting monthly one-on-one to assess progress.
- Assess, plan, and implement one on one and group health education programs as well as conduct evaluations and research, serve as a health education resource person and communicate and advocate for health, health education, health promotion and prevention.
- Represent the agency in community meetings, including and not limited to the Healthy Living Coalition.
- Participate in multi-disciplinary case conferences and morning huddles assisting in service delivery for each client.
- Maintain and integrate data from other CSC program databases with Circe.
- Conduct data QA/QI, development and ensure robust data cleaning protocols, and perform data validation.
- Support data analysis and reporting.
- Serve as an internal health and wellness resource for staff including designing healthy workplace programs, and/or create incentives for employees to lose weight, quit smoking or increase their level of exercise.
- Track units of service and outcome data.
- Supervise all grocery and food programs.
- Supervise the Community Health Worker program.
- Perform other duties as assigned.
JOB REQUIREMENTS:
- Minimum two years’ experience, knowledge and understanding of low-income and minority communities in San Francisco.
- Bachelors of Public Health, Health Education, or Health Science degree.
- Current Health Education Specialist (CHES) Certification.
- Proof of Covid Vaccines required: completed original series and first booster.
- Proof of negative or inactive TB required. Curry Senior Center will provide or reimburse for the cost of a TB test after an offer of employment has been accepted by both parties. Proof of a negative TB test within 90 days will also be accepted.

PHYSICAL DEMANDS:
The incumbent is in a non-confined office-type setting in which they are free to move about at will. The Environment is mostly clean and comfortable but may include some minor annoyances such as noise, odors, drafts, etc. Work will be performed utilizing a computer, sitting at a desk/chair or standing while speaking and interacting with clients. Position will require average finger dexterity to operate normal office equipment and systems.
