Overview
CVP is seeking Nurse Case Managers to join our team in providing our Nation's Veterans with healthcare services at the Office of Community Care in Glendale, CO. This is an on-site position with working hours Monday-Friday, 7:30 AM - 4:30 PM.
Location: Office of Community Care (OCC), 4100 E. Mississippi Ave, Glendale, CO 80246.
Duration: 5/1/25-4/30/26
Pay: $29.98-52.35/hr (Depending on Experience)
Health & Welfare: $4.93/hr (Can receive as cash in lieu of benefits)
Responsibilities
- Maintain patient/resident confidentiality and a safe patient/resident care environment, including familiarity with requirements such as medication administration, patient/resident/family education, discharge process, restraint usage, hazardous waste disposal, etc., as defined by The Joint Commission and all other VA regulatory bodies (i.e. Long Term Care Institute and Office of Inspector General).
- Clinically review consults/requests for completeness and appropriateness/utilization review.
- Disposition care in the community per utilization review and guidelines.
- Conduct clinical review of incoming medical records documenting communication with ordering providers and PCP.
- Coordinate care by corresponding with veterans, community providers, internal providers/nurses, and administrative staff to ensure requested care and authorization is processed.
- Attend daily team huddles and monthly staff meetings as required.
- Process individual view alerts in CPRS within seven days of creation.
- Demonstrate Integrity, Commitment, Advocacy, Respect, and Excellence (ICARE) values in all interactions.
- Maintain patient privacy and confidentiality in accordance with HIPAA and VA regulations.
- Provide patient/resident-centered care by performing the following tasks:
- Assess the health care needs of the patient/resident and maintain or develop a plan of care to meet interdisciplinary goals.
- Assess and act on significant changes in the patient’s/resident’s health condition.
- Carry out the plan of care by performing/assigning delegated tasks.
- Collect and document pertinent data into the electronic patient/resident medical record using designated temp plates with no personalized or customized documentation.
- Complete nursing assessments within required time frames, and in accordance with policy and procedure.
- Meet the health care needs of patient/resident by assessing and assisting the patient/resident to meet expressed needs.
- Prioritize and complete tasks within the RN’s assigned shift.
- Update patient/resident care plans.
- Notify assigned physician of any changes in condition.
- Practice the VA core values Integrity, Commitment, Advocacy, Respect, and excellence (ICARE).
- Report incidents of verbal or physical abuse immediately to the Nurse Manager of the unit they are assigned; or designated off-shift nursing leadership.
- Coordinate and participate in admission and discharge of patient.
- Obtain and deliver specimens to lab.
- Respond and assist in an emergency situation.
- PPE and Hand Hygiene Compliance.
- Provide Joint Patient Reporting System (JPRS) for patient incidents.
- Clear all CPRS clinical alerts daily.
Qualifications
- US Citizenship required to undergo government sponsored background investigation and credentialing.
- Applicants are required to have resided in the U.S. for the last 3 years.
- Graduate of a school of professional nursing approved by the appropriate State agency and accredited by the Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
- Current, full, active and unrestricted license as a Registered Nurse in at least one state, territory, or commonwealth in the U.S. or the District of Columbia, demonstrated by providing a copy of current licensure and applicable states.
- Minimum of two (2) years case management experience within the last three (3) years.
- Current Basic Life Support (BLS) certification required.
- Strong computer skills and ability to learn VA electronic health record systems.
- Experience with utilization review and care coordination.
- Ability to communicate effectively both verbally and in writing.
- Knowledge of medical terminology and clinical documentation standards.
