Job Description
We are seeking an experienced Travel Licensed Clinical Social Worker to join our team in Sitka, AK. As a key member of our multidisciplinary care team, you will play a vital role in assessing patients' biopsychosocial needs, managing discharge planning, and collaborating with Inpatient Care Coordinators.
Key Responsibilities
- Work closely with the inpatient care team to assess patients' needs and develop effective care plans.
- Manage discharge planning, ensuring smooth transitions for patients returning home or transitioning to subacute rehabilitation services.
- Perform swing bed assessments and facilitate patient/family care conferences to ensure understanding of care plans.
- Assist patients in making healthcare decisions and guide them in navigating the healthcare system.
- Identify community resources to support patients and family/caregivers during hospitalization and recovery.
- Support patients with financial or legal matters, long-term care or assisted living needs, dental, podiatric, eye care, and hearing services.
- Refer patients needing chemical dependence or mental health support to appropriate clinicians.
- Maintain detailed records in accordance with policy and regulations.
Requirements
- Master's Degree in Social Work (MSW) or Bachelor's Degree in Social Work (BSW)
- Valid State Driver's License (or obtainable within 6 months of hire)
- 2 years of experience in a hospital environment, preferably in social work
- Excellent communication, problem-solving, and conflict resolution skills
- Able to work effectively with diverse individuals and teams
