Job Summary
We are seeking an experienced Social Worker to join our team in Newark, DE. The successful candidate will work in a Level I trauma center, comprehensive stroke center, and regional centers of excellence in heart and vascular care, cancer care, and women's health. This role requires a strong foundation in acute care social work, with a focus on resolving psycho-social barriers and supporting discharge needs for our patients. Experience working with neonates, children, and families is preferred.
Key Responsibilities
This role involves:
- Collaborating with RN Case Managers and Utilization Managers to develop effective care plans;
- Providing education and support to patients and their families to ensure a smooth transition from hospital to home or other care settings;
- Maintaining accurate records and reports, adhering to regulatory requirements and professional standards;
- Participating in quality improvement initiatives to enhance patient outcomes and satisfaction;
- Engaging in ongoing professional development to stay current with best practices and industry developments.
Requirements
To succeed in this role, you will need to possess a Master's degree in Social Work (MSW) or a related field, and be licensed as a Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW). A minimum of 2 years of experience in acute care social work is required, with a focus on working with neonates, children, and families. Certification as a Certified Case Manager (CCM) within 18 months of eligibility is also desirable. Strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment, are essential.
