Must-Have: Proficiency with HomeCare HomeBase (HCHB) and Oasis, reliable transportation, current driver's license, and auto insurance

Job Description

• Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits and attend periodic office meetings.
• Handle cases including Start of Care, Wound Care, Follow-up Visits, etc.
• Manage an average of 5-8 patients per day, equating to roughly 6-8 points.
• Travel up to 75 miles from the home clinic; mileage reimbursed at the IRS standard rate.
• Attend both care facilities and patients' homes for appointments.
• Use EMR systems: HomeCare HomeBase, Oasis, and Outlook.
• Complete all documentation within 24 hours of the appointment with the patient.
• Participate in on-call rotations as required.
• Be open to accepting patients without refusal based on location and travel requirements.
