We are currently seeking a Social Services Director.
Position Summary:
The Social Services Director shall possess a four-year degree in Social Services or comparable field of learning; and at least two years’ experience in a health care setting. Any designee who does not possess these qualifications will receive consultation from a qualified social services consultant experienced in long term care. The Social Services Director must be able to recognize social and emotional needs of the residents and provide support to families. The Social Services Director must possess leadership abilities, strong oral and written communication skills, and be able to work independently.
Essential Job Duties:
- Serve as the resident/family advocate and will coordinate all psychological services involved in patient care from the time of admission until discharge from the healthcare center.
- Ensure the personal and property rights of all residents and conducts investigations, as necessary.
- Keep all matters confidential and will adhere to HIPPA regulations regarding Protected Health Information (PHI) of the residents and shall comply with local, state, and federal regulations in respect to his/her realm of responsibility.
- Be involved in family counseling, mood and behavior management programs, psychotropic medication monitoring and any psychosocial needs of the residents.
- Assist in interpreting various policies and procedures for residents and families and assist with Advance Directive Planning and end of life issues
- Maintain communication with appropriate staff and families regarding resident status and needs and will maintain sufficient records.
- Be considered an Essential Staff member who will report as this policy designates for emergencies, inclement weather, and crisis situations.
- Serve as Grievance Officer.
- Ensure Red Files of Life for the Independent Living residents.
- Serve as Care Buddy staff liaison, Care Plan coordinator, and a Heartfelt Connections trainer.
- Fulfill any other responsibilities as assigned by supervisor.
- Serve as Manager on Duty when scheduled.
Qualifications:
- Bachelor degree in Social Work, Gerontology or related field. Must be a licensed Social Worker (in current state of residence).
- At least 2 year’s experience, in a health care setting (preferably Long Term Care) working directly with residents and families.
- Excellent oral, written and interpersonal skills. Must present a professional manner and be able to deal with situations that may be stressful.
