Since 1960, D’Youville Life & Wellness Community has provided Greater Lowell and surrounding communities with Faith-Based, Nonprofit, Short-Term Rehabilitation and Skilled Geriatric Care earning the respect of its peers and the trust of the community at large. We are a 208-bed facility with 84-bed Special Care Dementia Unit.
We are seeking a Senior Housing Service Coordinator for our Independent Living Facility, Bruyere Gardens.
- Part-Time; 24 hours per week
- $20 to $28 per hour based on skills and experience.

Senior Housing Service Coordinator Qualifications:
- Bachelor’s degree or higher in Social Work, Psychology, Gerontology, Counseling or related specialty or significant work experience relevant to the position.
- Experience with AASC Online Software for the American Association of Service Coordinators preferred.
- Experience working in a HUD housing environment preferred.
- Basic experience with Microsoft Excel.
Senior Housing Service Coordinator Job Summary:
- Work with new and existing tenants to explain the service coordinator program.
- Identify, assess, select, develop and maintain referral relationships with local agencies.
- Refer and link tenants to service providers in the general community: case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, visiting nurse, preventative health screening/wellness and legal advocacy.
- Refer concerns about lease violations to the property Director.
- Develop case plans in coordination with assessment services in the community.
- Monitor ongoing provision of services from community.
- Facilitate tenant meetings, community-organizing and social activities.
- Support tenants to enhance the quality of their lives.
- Work with management in mediating conflicts between tenants.
- Establish tenant communications through meeting minutes, newsletter, flyer, bulletin board, etc.
- Set up volunteer support program with services organizations in the community.
