Overview
We are seeking a dedicated , dynamic and compassionate Memory Care Coordinator/ Director Assistant to join our team. The ideal candidate will play a crucial role in ensuring that patients receive the appropriate care and support they need. This position requires a committed indivudal with very strong organizational skills, the ability to schedule staff, attention to detail, communicate effectively with both patients and healthcare providers, and a commitment to enhancing the quality of life for those in our care both in memory care and assisted living.
Responsibilities
- Coordinate patient care services by collaborating with healthcare professionals, families, and community resources.
- Develop and implement clinical staff weekly schedules, manage memory care operations and help the director of resident care
- Help the director monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Provide direct support to patients, assisting with daily living activities as needed.
- Maintain accurate documentation of patient interactions and care plans in compliance with regulatory standards.
- Facilitate communication between patients, families, and healthcare teams to ensure continuity of care.
- Assist the director in case management duties, including scheduling appointments and follow-ups.
- Support patients in navigating healthcare systems and accessing necessary resources.
Requirements
- Previous experience as a nurse/lead medtech in a nursing home, home care setting, or similar environment is preferred.
- Knowledge of first aid practices is a plus.
- Strong background in clinical work, organizing staff and maintaining high levels or efficiency
- Ability to pass meds, audit carts and files accurately
- Experience in case management or patient care is highly desirable.
- Ability to assist with heavy lifting when required for patient mobility or support.
- Excellent interpersonal skills with the ability to build rapport with patients and families.
- Strong organizational skills and attention to detail are essential for managing multiple cases effectively.
- A compassionate approach towards assisting individuals with diverse needs, including childcare experience when applicable.

Pay: $20.77 - $21.93 per hour
