Highlights of the Memory Care Coordinator Position:
POSITION SUMMARY: The Memory Care Coordinator oversees the overall implementation, delivery and coordination of the Connections Memory Care neighborhood resident services, including engagement and supports the HWD in non-clinical health services. Coordinates engagement aspects of life for Memory Care residents including participating in the development of personalized plan or program for successful aging in community. Ensures residents satisfaction and achievement of their realistic goals. Motivates residents to function at highest level of independence.
ESSENTIAL DUTIES:
Promotes the company's Connections Memory Care Philosophy in all aspects of their roles to include interactions with residents, family and staff and the daily support of the oversight of operations in the Memory Care in partnership with the Health and Wellness Director/ Executive Director
Coordinates the designs and implementation of a robust engagement program for the residents in accordance with Connections Memory Care policies and procedures. Assists with delegating programming assignments to Care Partner team and provides support and training on programming.
Solely responsible for leading 75 % of daily programming while on duty. Collaborates with the Life Enrichment director for community wide events.
Support and participate in the implementation of all Connections Memory Care policies and procedures.
Acts as an advocate for Residents; assists in maintaining communication with designated agents as requested and required. maintains timely and open communication with family members to promote good relationships and keep them informed about resident's condition and community events; offers education, information, and support as needed.
Supports and/or facilitates the development of Monthly Support Group for resident families.
Maintains current Dementia best practices knowledge to support the growth and skills development of the staff.
Assists with ensuring programming service plans and interventions are personalized and confers with Wellness team on changes in resident condition.
Coordinates developing strategic non-pharmaceutical behavior interventions to support resident outcomes effectively.
Coordinates in assuring building, Residents units, common areas and adjacent grounds are maintained by working with Executive Director.
QUALIFICATIONS:
A passion for helping seniors. Must have a genuine interest in the care of the elderly and for issues that impact the elderly.
Life Enrichment, Activities Director Experience, Certified Dementia Practitioner, Certified Montessori for Dementia Practitioner qualifications preferred.
One and Three years of Memory Care experience, including programming, caregiving or leadership.
Knowledge of and/or ability to learn the theory and practice of Assisted Living and Dementia Care.
Demonstrated ability to communicate effectively (in English), both verbally and in writing, with residents and staff.
Must have a genuine interest in the care of the elderly and for issues that impact the elderly.
Must be able to cope with high levels of stress that position may cause.
Ability to relate to residents and staff in a courteous and diplomatic manner under all circumstances.
Demonstrated ability to work in a team setting and to convey strong public-relations skills.
Have effective organizational and project management skills with attention to detail
Possess excellent customer service skills.
