Job Summary: This position is responsible for supervising eligibility and case management staff supporting HIV services, ensuring the effective delivery of case management services for Daily Planet patients living with or at high risk for HIV. The supervisor will work closely with interdisciplinary teams, facilitating connections between high-risk patients and Ryan White services. This position will hold their own caseload varying between non-medical case management and medical case management to adjust to the team’s needs. The RW Case Management Supervisor will coordinate the patient caseload assignments, and oversee the day-to-day delivery of patient care coordination and case management services.
This role supports patients living with HIV in a treatment program, addressing and eliminating barriers to adherence with antiviral therapy to reduce the spread of HIV. Ryan White responsibilities are expected to comprise 100% of this position’s time, focusing on case management oversight, staff support, and program effectiveness. The person in this position will uphold the dignity, privacy, and confidentiality of patients and their families while fostering a team-based approach to patient care in collaboration with Daily Planet providers and staff.
Job Requirements: Bachelor’s degree in a relevant discipline required; Master’s degree preferred. Minimum of 5 years case management experience; 3 years of supervisory experience preferred. Must demonstrate knowledge of Ryan White Case Management standards and/or other relevant publicly funded programs. Experience administering public funding a plus. Ability to work independently and as part of a multidisciplinary team. Experience teaching adult learners. Experience working with patients experiencing homelessness and patients living with HIV. Skills include excellent oral and written communications including telephone, organizational, and personal computer including word processing and Microsoft office.
