What are the responsibilities and job description for the RN Care Manager position at Dameron Hospital?
POSITION SUMMARY:
Responsibilities include but are not limited to: Knowledge of state and federal guidelines and criteria for determining medical necessity for admission, level of care, utilization of resources and continued length of stay is required. Clinical knowledge of signs, symptoms, treatments, therapies, expected outcomes, and the discharge needs of patients in all age groups is necessary. The RM exercises discretion and independent judgment regularly in the performance of this position. They must display assertiveness and the ability to communicate effectively with patients, staff and physicians. The department will be staffed seven days/week; all staff may be asked to work weekends and holidays. The RM is responsible for promoting patient care continuity and quality through the collaborative development of practice guidelines and clinical pathways that support quality improvement activities. The RM has the responsibility, accountability and authority for providing comprehensive care coordination and knowledge to plan, implement, monitor and evaluate the outcomes of care for the designated patient population.
This position will be cross-trained in all areas of Resource & Utilization Management for maximum departmental flexibility. Must participate in the Respiratory Protection Program, which may include the wearing of a respirator. Must be able to perform jobs in all patient care and operational areas subject to reasonable accommodations, also be able to employ all mandatory safety devices and procedures. POSITION QUALIFICATIONS:
Education:
Education required to qualify for license as a registered nurse, Associate Diploma
BSN preferred.
Experience:
Minimum of 3 years in an acute care hospital, two (2) years of Case Mgmt experience preferred.
Licenses/Certificates:
Current California Registered Nurse License required
Current BLS Certification required
Certification in case management (ACM or CCM) preferred but not required
