We are looking for a dynamic and driven Home Health Marketer to join our team and help us grow our patient base. This role involves building relationships with physicians, hospitals, and other referral sources to generate leads for home health services. The Home Health Marketer identifies, develops, and maintains relationships between the agency and hospitals, nursing homes, physicians, and community organizations that result in the acquisition of patient referrals for the agency.

Essential Expectations and Procedural Responsibilities:
-Coordinate services for patients discharged from hospitals and other referral sources
-Obtain valid Medicare and other referrals as outlined in offer letter
-Identify patients PCP and obtain authorization for home health services
-Obtain doctor signatures for various home care orders and document accordingly
-Assist in development of patient care plan and coordinate home care needs
-Other duties as assigned by supervisor

Pay: $50,000.00 - $80,000.00 per year
