Description

Job Summary:

Under the direction of the Health Information Manager, the Health Information Coordinator supports the Health Information Department (HIM) by verifying and ensuring receipt of all patient records, and assembling the documentation into the legal medical record (EHR). Health Information Coordinator must at all times safeguard and protect the patient’s right to privacy by ensuring that only authorized individuals have access to the patient’s medical information and that all releases of information are in compliance with the request, authorization, company policy and HIPAA regulations.

Job Status: Full-time, Hourly Non-exempt

Duties/Responsibilities:

  • Updates Medical Records in EHR (Epic) in a timely manner.
  • Utilizes Document Management System on Base for indexing medical records.
  • Utilizes OCHIN scanning for scanning barcoded documents into Epic.
  • Completes tasks from Epic (in basket) in a timely manner.
  • Manages all administrative and clerical functions for the HIM Department.
  • Monitors and routes medical records, results and pharmacy requests, referrals, to providers or appropriate staff from incoming fax queues.
  • Follows up with requests for medical records.
  • Coordinates with Riverside County for supporting documentation for disease reporting.
  • Assists with My Chart patient medical records portal activations, password reset, and general support.
  • Releases consult records/procedure reports to referring provider’s offices in a timely manner.
  • Accesses medical records portals to retrieve records from hospitals, imaging facilities, etc.
  • Assists patients with filling out authorizations for third-party medical information access and release of information or any other HIM-related forms.
  • Ensures authorizations for the release of health information are HIPAA compliant and authorizations to requestors are in accordance with the minimum necessary policy.
  • Accesses patient-protected health information as required.
  • Answers telephone promptly and courteously within three rings and return voice messages.
  • Performs other duties as assigned.

Qualifications


Required Skills/Abilities:

  • Strong clerical and computer skills including proficiency with data entry and data collection systems.
  • HIPAA knowledgeable.
  • Proficient in working with Microsoft Outlook Suite.
  • Excellent communication skills – both verbal and written.
  • Excellent customer service skills.
  • Effective organizational skills are a must and be able to manage competing priorities.
  • Highly detail orientated.
  • Ability to:
    • Work cohesively in a team atmosphere with diverse populations.
    • Understand and become knowledgeable of the Release of Information Standards, Policies and Procedures, and HIPAA regulations, and complete work in compliance with these and other standards.
    • Maintain confidentiality of all patients and patient correspondence.
    • Take and follow through with delegated tasks and accountability.
    • Be a self-starter and self-motivated, or work independently.
    • Empathize with others and defuse conflicts; handle a crisis in a calm and respectful manner.
    • Exhibit resourcefulness in problem solving.
    • Operate agency EHR (Epic), Epic (Scanning) Document Management System (On Base).
    • Adapt to constant internal department changes.
    • Multi-task effectively.
  • Bilingual in Spanish, preferred.

Education and Experience:

  • A minimum of two years Health Information experience or in a health care related field.
  • Experience in EPIC, preferred.
