Your Role : Driving Patient Enrollment and Engagement
As the first Enrollment Specialist at Daymark Health, and as a member of our initial team, you will be the first point of contact for prospective patients, playing a pivotal role in growing our patient base and helping individuals understand the value of our services. You will engage with patients over the phone, educate them on the benefits of joining Daymark's program (at no cost), and guide them through the enrollment process. You will be instrumental in connecting patients to their dedicated care teams, ensuring they receive personalized support tailored to their needs.
What Success Looks Like
Within your first three months, you will be able to :
- Make 60 outbound calls daily to introduce Daymark's services, build relationships, and enroll patients into our program.
- Engage patients with empathy by actively listening to their needs, answering their questions, and building trust through genuine, supportive interactions.
- Surface patient goals and priorities , gathering insights that help our care team tailor support and resources effectively.
- Welcome new patients and caregivers , clearly explaining Daymark's care navigation process and the array of resources available.
- Manage inbound communications , acting as the first point of contact for phone calls and emails, triaging inquiries, and connecting patients with providers when necessary.
- Obtain patient consent and complete enrollment surveys, escalating urgent needs according to Daymark's protocols.
- Achieve and exceed performance goals , participating in coaching sessions and applying feedback to refine your approach.
