Pay Scale:
Extender: $16-$18/hr.
QP: $21.63-$23.07/hr.
Summary:
Under direct and indirect supervision, provides outreach to engage identified clients into Tailored Care Management (TCM) services.
Essential Duties and Responsibilities:
- Performs outreach to identified clients to educate and engage them in Tailored Care Management (TCM) Services.
- Keys in important client data to schedule appointments. Successfully link clients to center care management staff to begin care management assessment.
- May file, release and copy protected healthcare information. Answers phone calls, transfers to the appropriate parties, is courteous to the public.
- All other duties as assigned by supervisor.
The responsibilities of the Care Management Engagement Specialist include but are not limited to, the following:
Care Management Outreach/Engagement
- Performs outreach to identified clients to successfully engage them in Tailored Care Management (TCM)
- Utilize various communication methods to engage clients including phone, tele-health, patient portal, letters, etc.
- Provide education to clients on benefits of TCM
- Gather and key in important client data to schedule follow up appointments.
- Successfully link clients to center care management staff to begin care management assessment.
- Ensures all engagement activities are appropriately documented in Care Management dashboard
- Provide administrative support to centers as needed. May answer phone calls, transfer to appropriate parties, is courteous to public. May file, release, and copy protected healthcare information.

Education and/or Experience:
Extender Level- high school diploma or associate degree with experience working with adults or children with mental health or substance use disorder needs. or lived experience.
Qualified Professional Level- human services bachelors degree with 2 years experience with the population served or non-human service bachelors degree with 4 years experience with the population served.
