**Job Overview:**
The Delaware County Intermediate Unit seeks a highly skilled and dedicated Case Manager to join our team as part of the Project ELECT initiative. This 10-month position is an exciting opportunity for an individual passionate about supporting students in optimizing their functioning through knowledge of problem-solving, increased capacity, and effective linkages to resources.
**Key Responsibilities:
- Assist in the classroom, activities, and on-field trips to support student-clients.
- Compile, maintain, and submit required student data to monitor program progress.
- Conduct group and individual support sessions, as well as home visits, to ensure plan implementation.
- Develop and implement Individual Plans to ensure proper support for students.
- Nurture community networking and referrals within community-based organizations to improve and expand program services.
- Oversee student identification, outreach, recruitment, and intake to meet the needs and goals of the DCIU.
- Provide ongoing information and assistance to the program coordinator and colleagues to ensure program success.
- Represent Project ELECT and the DCIU at assigned meetings and events to communicate program goals and objectives.
Required Skills and Qualifications:
- Community College and/or Vocational School degree with study in a related field.
- Bachelor's degree preferred.
- Desirable job-related experience.
- Familiarity with community resources.
- Ability to keep accurate records and reports.
- Excellent verbal and written communication skills.
- Proficiency in computer usage.
- Reliable means of transportation.
