We are members of the Pennsylvania Association of County Affiliated Homes, and we are proud to serve as a geriatric training site for local colleges, universities and nursing schools.Fair Acres offers competitive compensation, great benefits- including a pension, a pleasant working environment, and the opportunity to make a difference!Position SummaryThe Resident Nurse Assessment Coordinator objective is the achievement of the highest quality resident care thru coordination and input according to the RAI Manual and CMS and State guidance along with adherence to the established procedures of the assessment process.ResponsibilitiesAccurate and timely completion of RNAC designated sections of the MDS on assigned unitsAssure completion of the MDS by other disciplines and report non-compliance to DirectorTimely signing of all necessary paperwork and coordinator of IDC meetingsAttend scheduled MDS / IDC meetings on assigned units or where neededParticipate in the evaluation process to determine the need for a Significant Change and implement Significant Change MDS and IDC meetingAccurate and timely scheduling and completion of discharge assessments and Death in Facility and entry trackersAssist in determining Medicare A Nursing skill upon admission and readmission; notify IDC team and RAD Department when skill endsAssist in completion of required documentationOversight of Care Plan completion and timely updatesNotify IDC team of changes to ARD or IDC dateProvide information for Managed Care updates and audits as requestedQualificationsGraduate of an accredited school of nursingCurrent PA Registered Nurse License1-2-year experience in Long Term CarePrevious Registered Nurse Assessment Coordinator experience preferredContactTo apply, please fill out our online application form.
