Job Description
As a Care Manager, you'll play a vital role in developing, assessing, and facilitating complex care management activities for our members. Your goal will be to provide personalized care plans and education to members and their families.
- Evaluate member needs and barriers to care
- Develop ongoing care plans and collaborate with providers
- Identify problems/barriers to care and provide interventions
- Coordinate care between member, family/caregivers, and care providers
- Provide ongoing follow-up and monitoring of member status
- Provide resource support to members and care managers
- Facilitate care management and collaborate with providers
- Perform telephonic, digital, home and/or other site outreach
- Collects, documents, and maintains member information and care management activities
- Provides education to members and their families/caregivers
- Provides feedback to leadership on opportunities to improve care
- Other duties as assigned
Required Skills and Qualifications
Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2-4 years of related experience. RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
