- Maintain active involvement in the individual child’s permanency goal process, working with the child’s caseworker/social workers; biological, foster, and kinship families; attorney; the Family Court system; mental health clinician(s); as well as other team members deemed necessary.
- Convey and interpret feedback from foster care families to the supervisor.
- Be involved in setting expectations and holding all foster care and adoptive families to those expectations.
- Provide professional consultation to foster care families to discuss methods or services needed to address the child’s needs and to be a supportive base. The permanency worker shall meet in the home of the family according to The Statewide Adoption and Permanency Network and The Department of Human Services Performance Standards.
- Provide written, appropriate material to consultative staff to document all work completed.
- Develop and review all the paperwork associated with the SWAN Unit of Service being performed including, but not limited to Child Preparation Plans, Meeting Activity Summary Reports, Individual Adoption/Permanency Service Plans, Child Profiles, Family Profiles, Finalization Paperwork, and Monthly Finalization Visit Notes.
- Follow-up on all referrals from Children and Youth agencies on at least a monthly basis.
- Make use of appropriate community-based agency support and keep accurate records of all referral contacts.
- Keep accurate records of all social service contacts and work with a case in the SWAN portal.
- Ensure that complete records are maintained for each child, including important documents such as Social Security cards, Medical Assistance Card, birth certificates and/or copies, baptismal certificates, medical/surgical consent forms, etc., initial intake form, social history, intake summary, psychological material, application, family service plan, individualized contract, ISP and quarterly evaluation forms, medical treatment plans, ongoing medical examination forms, immunization charts, correspondence, discharge summary, and any additional information pertinent to a child.
- Responsible for all correspondence with other agencies regarding the children in placement and their families.
- Team with direct casework services to children and their families in placement.
- Explore permanency possibilities for children where these alternatives are considered advisable.
- Participate in staff training.
- Responsible for notifying parents and authorities, including the Department of Children and Youth, in the event of a runaway or hospitalization.
- Notify Childline of suspected abuse.
- Effectively participate in the supervision process.
- Demonstrate ethical and professional conduct at all times.
- Promote continuous quality improvement projects and ongoing performance measures.
- Promote and address Delta’s Risk Management Program for the health and safety of individuals and employees.
- Perform other duties as requested.
- Knowledge of child welfare systems, SWAN services, and community resources
- Knowledge of trauma-informed care and implications for caregivers
- Demonstrate strong assessment and documentation skills
- Basic knowledge of the impact of substance abuse, sexual abuse, physical abuse, domestic violence, community violence, homelessness, poverty, etc. on child development and implications for caregivers
- Demonstrate effective crisis management skills
- Ability to form collaborative working relationships with diverse populations, including working with individuals of diverse cultures, ethnicities, gender, age, sexual orientation, socioeconomic status, religion, education background, etc.
- Ability to read, write, and speak English; perform arithmetical calculations
- Ability to use excellent oral and written communication skills
- Demonstrated commitment to diversity, equity, and inclusion
- Ability to use telephone, voice mail, copier, facsimile machine, calculator and other office equipment
- Fully proficient in Microsoft Office, software used in social services applications, and the ability to learn new and emerging technologies
- Ability to prioritize responsibilities and delegate duties to others
- Ability to work effectively with people, facilitating their growth and development
- Ability to use acquired knowledge of supervisory and administrative principles and methods
- Bachelors in Social Work, Psychology, Human Services, Counseling, or Criminal Justice required. Prefer some experience with knowledge of SWAN Services, Foster Care System, CUA, PA DHS, and DHS regulations.
- Master’s Degree in Social Work, Psychology, Human Services, Counseling, or Criminal Justice preferred.
- Experience working with families and children in community-based and home-based settings
- Current PA Child Abuse, FBI, and Criminal Clearances as well as child abuse clearances from any state lived in the past 5 years.
- Valid driver’s license in the state of residence.
