Responsibilities
Responsibilities include providing direct/indirect therapy services to outpatients who require or will benefit from therapy skills and interventions. This includes, but is not limited to:
Provides individual and group services.
Provides case management services to coordinate care with primary care doctors, other prescribing physicians, probation, parole, child/adult services, and other supporting services
Attends all clinic team meetings and company training
Communicates at a level that reflects above-average quality work
Maintain client records and other data as required by regulatory agencies promptly.
Treatment Planning: Applies assessment, evaluation, and reassessment data to the formulation of treatment plans. Supports recovery principles by incorporating the individual's and their family's (when applicable) strengths, barriers, and needs into the plan components. Documents problem statements and treatment goals/objectives/discharge plans that are written in clear and understandable language and include clients' own words. Formulates measurable, achievable goals and objectives within the scope of treatment provided in collaboration with the client. Describes clinical modalities/interventions that are responsible and provided according to applicable protocols. Reassess treatment effectiveness and adapt treatment plan as needed in collaboration with the individual client and treatment team.
Refers clients to hospital/designated mental health professional if no less restrictive option is available.
Maintains continuity among clinical teams by documenting and communicating actions, irregularities, and continuing needs
Making deliveries of medication as permitted by company insurance and DEA regulation
Occasionally lift and/or move up to 25 pounds.
