Position Title: Manager of Complex Case Team
Reports to: Director of Homelessness Resolution – Mobile & Community Connections
Job Classification: Full-time; Exempt
Salary Range: $57, 900 - $77,100 annually

The Opportunity:
The Manager, Complex Case Team position is responsible for directing the processes and staff for a team of individuals working specifically with people experiencing homelessness with complex high acuity needs and/or a chronic homelessness designation.
Responsibilities will include but not be limited to:
General Duties:
- Foster a culture of grace amongst staff that orients them around the mission of meeting individuals at their physical and spiritual points of need.
- Initiate and maintain good relations with all DRM departments.
- Maintain the relationships necessary to ensure DRM’s case management efforts reflect a real-time understanding of broader re-housing efforts, availability, requirements, and protocol.
Core Areas of Responsibility:
- Manage the staff experience with recruiting, interviewing, hiring, onboarding, training, resolving conflict, payroll, scheduling, time off requests and yearly reviews for staff on the Complex Case Team (CCT).
- Provide regular supervision of staff on the CCT by convening weekly team and individual meetings.
- Conduct case audits to ensure quality and consistency of staff documentation and case load engagement.
- Ensure data collection and reporting for outcomes is completed for external contract(s) and internal DRM requirements.
- Work closely with Managers on other DRM teams to ensure Continuity of Care within the internal DRM system.
- Guide team in processes to creatively and flexibly engage and problem solve high acuity clients’ barriers to accessing stable housing solutions. Strategize service delivery with a particular consideration of risk and complex problem solving.
- Ongoing audit of grant performance and development of processes and workflows.
- With the help of DHRMCC – oversee budgets and work with the accounting team in obtaining needed funds for Client Housing placements and other client services engaged by the CCT.
- Provide continual oversight and training to complex case management staff ensuring that they remain housing-focused, and client centered considering new ways of engagement and problem solving with intensive engagement.
- Ensure that all complex case managers are entering the appropriate data into Homelessness Management Information System (HMIS) and SCRIBE regarding the individualized housing plans and assessments to facilitate outcome reports to both internal and external partners.
- Work with DRM data specialists to develop processes and prompts within SCRIBE to facilitate proper recording and coordinate efforts toward HMIS training and reporting.
- Lead internal DRM Complex Case meetings to discuss internal referrals and problem solve what client needs to be able to maintain shelter services.
- Build collaborative relationships with enforcement entities, hospital systems, Medicaid related services, and Behavioral Health agencies.
- Attend community/internal DRM meetings that promote collaboration.
- Attend weekly meetings with key staff members and case managers that promote collaboration.
- Represent the Mission at meetings and on committees as directed.
- Communicate with the public through speaking engagements, community meetings, interviews, etc.
- Other duties as assigned.
*Schedule will include early mornings or late evenings or both.
Requirements
MINIMUM QUALIFICATIONS:
- Bachelor’s degree required. A combination of education and experience can be substituted.
- Three (3) years’ experience serving underserved populations.
- Preferred experience in managing employees in case management tasks.
- Knowledge in Behavioral Health approaches including Trauma Informed Care, Motivational Interviewing, Crisis Intervention and De-escalation strategies.
- HMIS experience preferred.
- Experience in understanding Home and Community-Based Services (HCBS) and the long-term care (LTC) process would be helpful.
- Must have an Active Driver’s License as a limited amount of work-related travel may be required.
- Must be willing to drive clients to appointments.
- Must be a Christian holding to the beliefs set forth in the Statement of Faith and Apostles Creed, expressed by a personal testimony and Christian conduct.
