SUMMARY OF DUTIES
This position assesses initial applicants, their family situation, and on-going participants to develop, coordinate, and arrange a variety of services to allow elderly and disabled adults or medical patients about to be discharged from a hospital to get the continuity of care needed or to allow families in Oregon's family assistance programs to become self-sufficient while helping them to meet their basic needs.

Major duties include:
- Perform reassessments (CA/PS), at least annually, on all service cases and complete within designated timeframe to avoid interruption of services and payment to providers. Determine if modifications of service plan are needed and complete paperwork related to any changes in facility or provider hours. Complete review of financial eligibility at least annually and document as required by policy.
- Provide monthly direct/indirect contact with clients, pulling and utilizing computer-generated reports, Targeted SNAP reviews and other reports. Make adjustments to care plans and vouchers, facilitate the homecare worker selection process and request vacation voucher payments to homecare workers as needed. Narrate pertinent client contacts in OACCESS within designated timeframe and in a manner that demonstrates ongoing monitoring of safety/health and services.
- Coordinate Community Health Support Program (Long Term Care Community Nurse) and Emergency Response System services for clients as needed.
- Coordinate with Diversion-Transition Coordinator immediately following notification of client going into NF care. Pursue all avenues toward relocation for any resident with the potential to return to the community.
- Work with intensive assessment case manager on difficult clients for coordination of placement or Behavioral Support Service referrals.
WORKING CONDITIONS
The worker is required to make home visits to various areas of Douglas County. Case Managers may be exposed to unclean living conditions, communicable diseases, second-hand smoke and pets. The position requires regular contact with clients/family members/other contacts who may be emotionally fragile, demanding, and potentially hostile. The ability to prioritize vulnerable clients, think critically in crisis situations, and communicate appropriately in stressful situation is essential.
There is required use of a laptop computer during home visits; strong computer skills are required to navigate a variety of mainframe screens and search online for pertinent rules and regulations pertaining to eligibility and program specifics. Multiple and restrictive Federal and State regulations apply. Numerous procedures exist for processing assignments that vary in nature and must be completed timely and accurately. The worker must be able to communicate program requirements in a manner that facilitates understanding to the general public and mentally and/or physically impaired individuals. Physical demands of the position in the office are limited.
MINIMUM QUALIFICATIONS
- A bachelor's degree in a Behavioral Science, Social Science, or a closely related field; OR
- A bachelor's degree in any field and one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing); OR
- An associate's degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing); OR
- Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
