Your Key Responsibilities
- Patient Assessment: Perform comprehensive evaluations to determine patients’ home care needs.
- Care Planning: Create and implement personalized care plans tailored to each patient’s unique requirements.
- Team Coordination: Lead and guide care teams, ensuring all members are informed and aligned with patient care goals.
- Counseling: Educate and support patients and their families, providing healthcare instructions and guidance.
- Documentation: Maintain accurate clinical notes and communicate updates to primary physicians.
- Licensure: Current and valid Arizona State Board of Nursing RN License.
- Experience: Minimum of one (1) year in home health care with proficiency in OASIS documentation.
- Transportation: Valid Arizona driver’s license and access to reliable transportation.
- Certifications: Current CPR certification and Arizona Fingerprint Card.
- Language Skills: Bilingual or conversational Spanish skills are highly desirable.
- Degree: Minimum of an Associate's Degree in Nursing.
