Key Responsibilities
- Assess patient and family needs, providing care in alignment with physician orders and interdisciplinary care plans.
- Collaborate with the assigned Nurse Case Manager to effectively assess and manage patient care.
- Communicate with attending physicians, team members, and external agencies to ensure coordinated care.
- Maintain regular communication with the Nurse Case Manager, Program Director, and Medical Director regarding patient progress.
- Collect data on physical, psychological, social, and spiritual factors affecting patient health and report findings.
Qualifications
- Licensure: Current LPN/LVN license in the State of Arizona.
- Availability: Willing to provide some after-hours and weekend coverage.
- Skills: Strong verbal and written communication skills, with excellent organizational abilities.
- Requirements: Licensed driver with a reliable, insured vehicle, current BLS certification, and Arizona Fingerprint card (or the ability to obtain one).
- Experience: Home health experience is a plus, but not required.
- Language Skills: Bilingual in English and Spanish is required.
