What You’ll Do:
Coordinate Patient Intakes: Collect and process referral and medical information quickly and accurately
Partner With Providers: Liaise with physicians, nurses, hospitals, and other care professionals to ensure seamless care
Verify Insurance and Authorizations: Confirm coverage, obtain authorizations, and prevent delays in service
Keep Records Up-to-Date: Enter, track, and maintain patient data in EMRs and other systems
Ensure Compliance: Follow all HIPAA guidelines and protect patient privacy at every step
Support Patients and Families: Provide clear, respectful communication and guide them through the process
What You’ll Bring:
Education: High school diploma required; associate’s or bachelor’s degree in healthcare preferred
Experience: At least 2 years in a healthcare setting, with a strong preference for Home Health experience
Knowledge: Familiarity with medical terminology, insurance processes, and healthcare regulations
Tech Savvy: Comfortable using EMR systems and data tools to manage sensitive patient information
Strong Communicator: Confident working with patients, providers, and internal teams
Detail-Oriented: Organized and meticulous when handling documentation
Flexible & Adaptable: Able to shift priorities and take on occasional evening or weekend tasks as needed
