Role Overview
As a Hospice Medical Social Worker, you’ll play a vital role in providing emotional, social, and practical support to patients and families during life’s most challenging times.
Key Responsibilities
- Assessment: Evaluate patients' and families' psychosocial needs and collaborate with the interdisciplinary team.
- Counseling: Provide support and crisis intervention to help patients and families cope with stress and emotional challenges.
- Education: Assist with advance directives and explain patient rights and available resources.
- Collaboration: Partner with community agencies to enhance support and care for patients.
- Care Planning: Participate in interdisciplinary meetings to develop and implement personalized care plans.
Qualifications
- Education:
- Master’s degree in Social Work (MSW) from an accredited program, OR
- Bachelor’s degree in Social Work with additional qualifications per Medicare guidelines.
- Experience: At least 1 year in healthcare; home health or hospice experience is preferred.
- Skills:
- Excellent verbal and written communication.
- Strong organizational skills.
- Deep understanding of hospice philosophy and end-of-life care.
- Licensure: Current state licensure, a valid driver’s license, and access to a reliable, insured vehicle.
