What are the responsibilities and job description for the RN, Oasis Specialist position at Desert Oasis Healthcare?
The RN, OASIS Specialist will be responsible for assisting management with collecting, analyzing and auditing OASIS documentation to improve operational processes. This position will assess processes and outcomes to ensure the highest level of quality care, utilization and documentation standards.
- Review documentation for completeness and accuracy while utilizing Outcome and Assessment Information Set (OASIS) data findings to identify areas of opportunity and process improvement.
- Track and trend findings for presentation to Quality Manager and Quality Committee.
- Provide 1:1, group and agency-wide education regarding findings and enhancement opportunities.
- Act as back up to Operations Manager to ensure OASIS assessments are transmitted and locked within mandated timeframes.
- Provide support as the internal subject matter expert on OASIS related issues.
- Monitor Quality of Patient Care (QoPC) Star rating and Patient Survey Star ratings to recognize where process improvement needs to be implemented.
- Serve as a member of the telehealth team to assist in remote patient monitoring and intervention.
- Assist with telehealth equipment setup and troubleshooting technical issues. Serving as a liaison between vendors and patient(s).
- Assist with referral reviews and workflow, as needed.
- Perform other duties as assigned.
- Associates Degree in Nursing.
- Current California Registered Nurse (RN) license.
- 1 year of Home Health experience, preferred.
- Knowledge of the OASIS Assessment, preferred.
- Excellent critical thinking skills.
- Ability to effectively communicate with individuals within all levels of the organization, patients, patient family members, providers, vendors and others.
- Ability to manage time effectively and prioritize tasks to meet established deadlines.
- Current California driver's license and proof of auto insurance.
- At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
