Your Role as a Chaplain

· To assess and document the patient/family’s religious/spiritual needs and resources on the intake/assessment form and to complete progress notes in the patient’s chart following each contact.

· To provide, when invited, direct spiritual counsel, prayer, and sacraments to patients and their families.

· To interface with the patient/family’s clergyman, rabbi, or their congregation, if requested.

· To encourage patients and families to make arrangements in advance regarding the funeral, wills, requests, obituaries, and interment, and to be available to officiate at funeral services if requested.

· To attend and participate in scheduled patient care conferences, Quality Improvement Meetings; staff meetings, and all other applicable meetings and in-services.

· The Chaplain is required to provide a point of service electronic charting and documentation at the time of patient visit or admission. Ability to operate a computer and/or a tablet PC, and proficiency with Windows-based software programs, is required.

· Other duties as assigned by the Chaplain Supervisor

Actively participates in quality assessment performance improvement teams and activities.

Experience

• Personal /professional experience working with death and dying

• Familiar/comfortable working with varied spiritual/religious practices

• Self-starter, personable, accountable, attentive to detail; able to provide active listening and compassion.

• Bilingual English/Spanish preferred

Required license or certification: * Master of Divinity, Theology, or equivalent required. 2 CPE units. Pastoral Care experience required.

Salary Range- $28.00-32.00

Job Types: Full-time, Per diem

Pay: $28.00 - $32.00 per hour

Expected hours: 40 per week

Schedule:

• 8 hour shift
• Monday to Friday
• Weekends as needed

Work Location: On the road
