What are the responsibilities and job description for the Per Diem Chaplain for Hospice Care (Barstow) position at Lake View Hospice?
Responsibilities:
- Provide spiritual and emotional support to patients, families, and caregivers receiving hospice care.
- Conduct assessments of patients’ spiritual needs and offer personalized care aligned with their beliefs and values.
- Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
- Offer grief support and counseling during end-of-life transitions.
- Conduct or facilitate religious rituals, ceremonies, or prayer sessions as requested.
- Respect and accommodate diverse spiritual, cultural, and religious backgrounds.
- Document services provided in compliance with hospice policies and regulations.
- Provide education and resources to families regarding coping strategies and spiritual wellness.
Requirements:
- Bachelor’s or Master’s degree in Divinity, Theology, Pastoral Counseling, or related field.
- Ordination or endorsement from a recognized faith tradition.
- Experience in hospice, palliative care, or healthcare chaplaincy preferred.
- Strong communication and interpersonal skills.
- Ability to work effectively with individuals of diverse spiritual and cultural backgrounds.
- Valid driver’s license and reliable transportation for travel within the Lancaster, CA area.
