Developmental Pathways (DP) is seeking a Case Management Specialist (CMS) in our Program Quality Department. This Case Management Specialist will provide professional level administrative support to a variety of roles and work processes. The Case Management Specialist will develop and maintain systems of organization, distribution, and communication. Our ideal candidate assists and support staff with the administrative execution of their job duties as well as possess strong organization, collaboration, critical thinking, and communication skills.
This is a 40-hour per week, hybrid position, with the benefit of working remotely and in-person coming into our headquarters office in Aurora, CO and/or off-site locations

Requirements
-
High School Diploma or GED
Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.
Preferred

Associates Degree in business equivalent education and experience

Three years (3) administrative support or customer service experience
-
Experience in human services or nonprofit fields

Knowledge of the Colorado Department of Health Care Policy and Financing and the Colorado Department of Public Health and Environment
Additional Responsibilities
-
Develop, review, distribute, and manage service plan (SP) and billing data for service providers as needed; enter SP data into state databases as needed; support and resolves billing issues as needed

Ensure accuracy and timeliness of information entered into internal and external databases and tracking systems

Research participants’ service utilization and previously provided services and supports and follows up as appropriate

Analyze work completed to identify and report on trends

Provide professional level feedback and follow up to other professional level staff regarding mistakes or missed steps in department processes and procedures (e.g., for SP and revision completion and entry into PAR system)

Keep up to date on all state policies and procedures related to their tasks and responsibilities

Operate independently, using Health Care Policy and Financing rules, regulations, policies, and procedures, as well as directives from the Associate Directors, Program Managers, and other senior leadership to provide technical guidance to internal and external stakeholders to generate solutions to problems, or coordinate with other staff for resolution and follow up

Communicate and coordinate with both internal and external partners professionally and thoroughly

Collaborate effectively on projects; support teams in developing and improving work processes

Coordinate additional administrative processes, including purchasing products for individuals in services, provider relations, Requests for Proposals, Support Levels, and Long-Term Home Health PARs, as assigned
