Provide in a professional manner medical supports for individuals as directed by supervisor and/or a DSNWK nurse. The advocacy and treatment of individuals served with dignity and respect is a prime responsibility.

Essential Functions:
Participate and/or provide information in the life planning process for individuals as requested by Nursing Services.
Maintain a cooperative and professional relationship among teams/peers by communicating information; responding to requests; building rapport; participating and contributing in team problem-solving methods.
Promote individual's independence by addressing medical treatments, assisting in understanding conditions, medications, self-care skills; answering questions as appropriate. Communicating these to Nursing Services for follow-up.
Protect individuals and other employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
Practice appropriate protective and preventative health measures for individuals served as per agency policy and other applicable regulations.
Practice the safe and appropriate administration, storage, labeling and record keeping of medications. Ensure prescribed medications are available and ready to be administered.
Assist in scheduling, providing transportation and attending medical appointments as outlined in each assigned individual's life plan.
Ensure Nursing Services is advised of doctor appointment results and responsible on-duty staff are made aware of medication needs.
Provide teaching to individuals served on medical issues as directed by the agency nurse, and approved by supervisor.
Attend initial and ongoing training in a timely manner to meet the needs of individuals served and meetings as requested by supervisor.
Maintain documentation of individual's care and treatments that complies with DSNWK policies, as well as federal and state regulations. Complete all daily/monthly paperwork and documentation in a timely manner.
Participate in crisis intervention including physical restraint, lifting and transferring of individuals served, according to agency policy, as necessary.
Familiarize self with the rights of individuals served and ensure these rights are maintained at all times through appropriate verbal and physical interactions.
Familiarize self with and implement the risk management plan for each individual served.
Report health, safety, licensing and abuse/neglect/exploitation (ANE) issues to appropriate personnel per agency policy.
Monitor individuals health status and report changes to Nursing Services.
Ensure individuals served are treated with dignity and respect to include using a respectful tone, giving full consideration and attention to each person served and providing opportunities and options that promote independence.
Practice, monitor and ensure quality teaching of skills to individuals served to include but not be limited to recognizing opportunities to teach, providing rationales, offering positive reinforcement and encouraging decision making skills by each individual to promote independence.
Maintain assigned cleanliness and Report any vehicle maintenance issues to secretary or transportation.
Obtain and remain current in providing Non-Emergency Medical Transportation (NEMT).
Ensure adequate amounts of medical supplies are on hand, submit requisitions when they are needed.
Provide assistance with Activities of Daily Living (ADLs) to include support and supervision during medical appointments and transportation etc.
Complete other duties as assigned by the Nurse Manager and Director of Programs and Services.
Other Requirements:
Maintain technical knowledge by attending educational training as required by DSNWK and the State of Kansas.
Maintains confidentiality and protects operations by keeping information confidential.
Administering Medication and Treatments, Verbal communication, Listening to understand, Infection Control, Medical Teamwork, Multi-tasking, Health Promotion and Maintenance.
Accompany individuals served to hospital to assist with admission/discharge, when appropriate
Implement crisis intervention strategies including physical restraint, lifting and transferring of individuals served according to agency policy as necessary. Attend Mandt Training.
Coordinate and attend medical appointments and facilitate communication between individual(s), Nursing Services and the medical professional(s).
Ability to use computers and various programs for documentation.
Hearing and visual capabilities must allow individual to perform essential functions.
Desire to work with adults with developmental disabilities.
Able to exercise good judgment and discretion in handling confidential information.
Valid driver's license.
First Aid/CPR Certified. Will Train.
This position will have access to PHI and EPHI for individuals whom medical and medication supports are provided.
This position is subject to the two year training period as defined by KPERS, unless exceptions are granted due to prior employment.
Job Specifications:
Education: High School diploma or equivalent required. Certified Nurse Aide and/or Certified Medication Aide in the state of Kansas, preferred but not required. Will Train.
Experience: Prior personal or job related exposure to people with developmental disabilities is preferred.
Physical requirements: Must be physically able to evacuate persons served in the event of an emergency and be able to apply necessary behavior management techniques as directed by the individual's Life Plan. Position may have other physical requirements for specific work sites, i.e. climbing stairs, extended periods of standing, etc.
