Responsibilities
- Conduct comprehensive patient assessments to develop individualized care plans.
- Provide health coaching to patients and their families, promoting wellness and self-management.
- Manage acute care situations, including the administration of tube feeding and other necessary interventions.
- Collaborate with multidisciplinary teams to ensure effective discharge planning for patients.
- Participate in clinical research initiatives aimed at improving patient care practices.
- Maintain accurate data management systems to track patient progress and outcomes.
- Support the daily operations of the Level I Trauma Center, ensuring adherence to safety protocols and standards of care.
- Engage in continuous education and training to stay updated on best practices within the field.
