We are seeking a dedicated and compassionate Case Manager to join our team. Duties
- Conduct comprehensive assessments of clients' needs and develop personalized care plans.
- Provide ongoing support and counseling to clients dealing with addiction, mental health issues, or other life challenges.
- Facilitate access to resources such as workforce development programs, healthcare services (including Medicare), and community support.
- Mentor clients through their recovery or rehabilitation process, fostering independence and resilience.
- Supervise casework staff and interns, ensuring adherence to best practices in client management.
- Collaborate with other professionals in child welfare, behavioral health, and crisis intervention to coordinate care.
- Maintain accurate documentation of client interactions, progress notes, and case outcomes.
Requirements
- Bachelor's degree in Social Work or a related field; relevant certifications are a plus.
- Experience in workforce development, addiction counseling, or behavioral health preferred.
- Strong skills in crisis management and intervention techniques.
- Excellent communication and interpersonal skills for effective mentoring and supervision.
- Ability to work collaboratively with diverse populations and multidisciplinary teams.
- Knowledge of child welfare systems is beneficial but not mandatory.

Pay: $19.27 - $21.17 per hour
