PURPOSE AND SCOPE:

The Corporate Quality Outcomes Specialists is responsible to coordinate, plan and manage the Corporate Clinical Quality Goals/Metrics for the entire company that will ensure the company meets or exceeds all CMS required, QIP, NHSN, and DCC quality goals.

Supports the mission, core values and customer service of Home Dialysis Services & Dialysis Care Center, LLC (HDS/DCC). Adheres to the HDS/DCC Policies and Procedures and Compliance Program including all state and federal regulatory requirements.

PRIMARY DUTIES / ACTIVITIES:

Monitor facility quality outcomes, educate, and provide interventions to staff on any outliers or metrics that is not meeting DCC Quality Goals.

1.Monitor Anemia Management


  • Implement anemia management protocols.
  • Review parameters and results to evaluate effectiveness of the facility Anemia Management PhySoft Program

• •Review new patients’ admission labs and initiate anemia management protocols as appropriate and as directed by physicians

  • Review all Hb/Ferritin/T-Sat results and implement dose adjustments for anemia medications per established protocols vis PhySoft Software for all initial and continued therapies
  • Maintain proactive communication with facility staff regarding patient specific conditions, events, etc. related to the Anemia Management PhySoft Program
  • Train, coach, and mentor new RNs on basics of PhySoft anemia management and anemia management protocols
  • Communicate with Clinical Nurse Managers/Area Manager via calls, video calls, emails, or physical meetings, regarding their facilities anemia management and interventions put in place.

2. Monitor Dialysis Access Dashboard

  • Follow up with CVCs company-wide utilizing the Vascular Access Dashboard.
  • Educate/suggest ideas to nurses on CVC removal for patients listed on the Dashboard during weekly outcomes call, via email, or physical meetings.
  • Communicate with clinic Clinical Nurse Manager/Area Manager/ Vascular Access Lead (VAL) regarding patients who have had a CVC>90 days to assist with permanent access.

  • Educate Staff on DCC Fistula First Initiative.
  • Educate staff on DCC Vascular Access Protocols and Standing Orders.

3. Track and Monitor All Modalities Adequacy

  • Educate/suggest ideas to nurses on any patient is less the Kt/V goal during weekly outcomes call, via email, or physical meetings.
  • Verify dialysis prescription is delivered as ordered by the physician.
  • Review and reinforce correct lab collection technique prior to each monthly draw.
  • Review lab results with the Clinical Nurse Manager, Dietitian, and Charge RN. Help evaluate outliers and plan interventions prior to the next dialysis treatment.
  • Verify data errors (e.g. transposed pre-post weights, wrong BFR, Target Weight, and treatment time) are corrected or data transmission problems are resolved.
  • Ensure the corrected data is corrected in EMR the same day labs are available.
  • Educate staff on DCC Adequacy Protocols and Standing Orders.

4. Track and Monitor Infections on the DCC Infection Dashboard

  • Review infection-related data for accuracy and thoroughness.
  • Educate/suggest ideas to nurses on any patient with infections during weekly outcomes call, via email, or physical meetings.
  • Review and track infectious trends of patients on the Dashboard
  • Ensure Data entered is accurate in EMR and Laboratory.
  • Actively participate in continuing education for Infection Control
  • Provide resources and training for both staff and patients in regard to infection control
  • Coordinate Infection-related Root Cause Analysis Activities and collaborate on plans for improvement with Clinical Nurse Manager
  • Educate staff on DCC Infection Protocols and Standing Orders.

5. Track and Monitor DCC IDWG Dashboard

  • Reviews the DCC IDWG Dashboard and discuss fluid interventions on the weekly outcomes meeting call.
  • Ensure Clinical Managers have implemented DCC protocols (if approved by Governing Body) and other fluid management orders standing orders.
  • Educate staff on the process for patient Target Weight (TW) Assessment
  • Assess blood pressure (BP), BP meds adherence, interdialytic weight gain (IDWG), Pre-Post Weight and obtain orders for TW adjustments as indicated.
  • Educate and train staff on when to schedule extra treatments or if TW challenge is indicated.
  • Educate staff on DCC Fluid Protocols and Standing Orders.

6. Track and Monitor Missed Treatments and Rescheduling Process.

  • Educate staff on DCC Missed Treatment Protocols and Standing Orders.
  • Educate staff on DCC Patient Rescheduling Process.
  • Reviews the DCC Missed Treatment Dashboard and discuss missed interventions on the weekly outcomes meeting call.
  • Reviews the DCC Missed Treatment Dashboard and discuss rescheduling interventions on the weekly outcomes meeting call.

7. Track and Monitor other Quality Goals/Metrics as assigned.

8. Other duties, as assigned.


9. Meet with the Director of Operational Efficiencies on a regular basis to discuss facility outliers and discuss interventions needed.

10. Travel will be required up to 40% to clinical facilities.

11. Travel to corporate office is required.

EXPERIENCE AND REQUIRED SKILLS:

  • Must be PC proficient and able to thrive in a fast-paced setting; experience in Microsoft Office Suite: Outlook, Word and Excel a must
  • Accountable for outstanding customer service to all staff, clients and vendors
  • Develops and maintains effective relationships through effective and timely communication.

OTHER QUALIFICATIONS

  • Must be a self-starter who can work independently yet function in a highly collaborative, fast-paced team environment.
  • Ability to multi-task, work under pressure and meet deadlines effectively
  • Excellent written and oral communication skills
  • Strong data entry, organizational and interpersonal skills
  • High integrity, team player with patient service focus and flexibility

EDUCATION:

  • Minimum of 3 years of dialysis nursing experience required
  • Graduate of an accredited School of Nursing, RN or BSN
  • Current appropriate state licensure as a registered nurse
  • Current CPR card

Day to day work includes desk and personal computer work and interaction with prospective and current clients. The position requires travel between assigned facilities and various locations. Travel to scheduled staff meetings may be required.

The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. The work environment is also characteristic of a dialysis center or a patient’s home and as such the employee may be exposed to infectious and contagious diseases/ materials.

Must be able to lift 30 pounds as needed.
