In this role, you will be instrumental in managing the scheduling and coordination of care for children and families, ensuring that all appointments and services are aligned for optimal health outcomes.

Key Responsibilities:
- Scheduling & Appointment Coordination:
- Manage and coordinate a variety of appointments for children and families, ensuring that they are scheduled efficiently and in alignment with care plans.
- Communicate with healthcare providers, therapists, specialists, and families to confirm appointments, send reminders, and ensure punctuality.
- Troubleshoot scheduling conflicts and work to find timely solutions that minimize disruptions to care.
- Care Plan Execution:
- Ensure that care plans are followed by scheduling appropriate services such as medical appointments, therapies, and home healthcare.
- Collaborate with families to prioritize and accommodate appointments based on their needs and preferences.
- Family & Provider Communication:
- Serve as the primary point of contact for families to discuss scheduling concerns, changes, and adjustments.
- Maintain strong communication with healthcare providers to ensure all appointments are on track and no critical services are missed.
- On-Call Responsibilities:
- Participate in the on-call rotation (once every 5 weeks), providing support for urgent care coordination needs outside of regular business hours.
- Receive additional pay for on-call shifts.
- Documentation & Reporting:
- Maintain accurate records of scheduled appointments, follow-ups, and changes to care plans.
- Provide regular updates and reports to the care team regarding scheduling and any challenges encountered.
- Crisis Management:
- Address urgent scheduling needs as they arise, ensuring that families receive timely care, particularly in times of emergency or crisis.
Qualifications:
- Education & Experience:
- Bachelor's degree in Social Work, Business, Public Health, or a related field (preferred).
- Experience in care coordination or scheduling within a healthcare or social services setting (preferred).
- Familiarity with coordinating appointments for children with special needs is a plus.
- Must be Fluent in Spanish and English
Skills & Abilities:
- Exceptional organizational and time-management skills, with the ability to juggle multiple appointments and tasks simultaneously.
- Strong communication skills to effectively liaise with families and healthcare providers.
- Ability to work under pressure and adapt quickly to changes in scheduling.
- Knowledge of medical and therapeutic services and terminology is an advantage.
- Bilingual candidates (English/Spanish) are encouraged to apply.
