Intake & Discharge Coordinator (Excelsior)
This is an on-site position based in Inglewood, CA. 
The schedule is Sunday through Thursday from 9:00am - 5:00pm PST.

The pay range for this role is $21.38 - $24.59 per hour.

Summary

As an Intake & Discharge Coordinator, you will serve as the lead over the intake and discharge process at the crisis residential facility, screening clients, interviewing clients, intaking clients, and completing discharge process.

 

Primary Duties

  • Making contact with referral sources and potential patients.
  • Conduct initial assessment and determine appropriateness for facility.
  • Ensures that each person served receives an appropriate orientation to the facility and types of services provided upon admission.
  • Monitors and maintains contractual and financial responsibility by continuously reviewing and tracking client enrollment patterns to make recommendations for programmatic needs.
  • Complete discharge packets to ensure patient has pertinent plan of care information.
  • Engages patients and responsible parties in the discharge process.
  • Completes documentation that is pertinent to discharge transition
  • Completes and documents descriptive arrangement of treatments and other community resources for follow –up services. i.e. dates, names, address and service providers.
  • Conduct post-discharge follow-up to reduce readmissions.
  • Monitors and maintains documentation of client services, ensuring compliance with Agency policies and procedures, Department of Mental Health contract and other related state/federal agencies’ standards and guidelines.
  • Develops and maintains care coordinator skills through seminars, workshops, or other forms of training/education related to working with adults with a history of mental illness and/or substance abuse.
  • Attends and participates in program, Agency staff meetings; and related community agency meetings as required/requested.

Position Requirements

  • Bachelor’s degree in Psychology, Social Work, Sociology or related area of emphasis.
  • 3 years experience working with adults with a history of mental illness and/or substance abuse in a social service/mental health environment.
  • Comprehensive knowledge of Evidence Based Practices, Co-Occurring Disorders and/or the Recovery Model of treatment is required.
  • Advanced verbal and written communication skills in order to provide high quality customer service and documentation of services with clients.
  • Comprehensive knowledge of crisis management techniques
  • Proven ability to exercise sound judgment with minimal supervision.
  • Know and comply with Agency policies and procedures, HIPAA, Department of Mental Health, policies and documentation guidelines, and other state/federal regulations relating to service programs for adults with a history of mental illness and/or substance abuse.
  • Efficiently use the personal computer to include a variety or word processing software.
  • Ability to utilize analysis, experience, and judgment to make decisions.
  • Demonstrated commitment to team objectives and Didi Hirsch philosophies
  • Adapt to changing needs by acquiring new skills and knowledge.
  • Current California driver’s license and a driving record acceptable to the Agency’s insurance carrier.
