POSITION SUMMARY:
The Referral Coordinator is responsible for managing internal and external referrals to mental health programs, ensuring timely and appropriate access to services for clients. This role involves coordinating with referral sources, program team members, and clients to streamline the intake process, maintain accurate documentation, and provide excellent customer service. The Referral Coordinator also plays a key role in educating potential clients, community partners, and providers about available services.
EDUCATION EXPERIENCE/REQUIREMENTS:
- High School Diploma, with at least 2 years of experience in mental health, social services, or healthcare coordination. Familiarity with referral systems and intake processes is a plus.
- Experience with Avatar required
- Knowledge of Insurance and funding sources
