What are the responsibilities and job description for the Person-Centered Options Counselor position at Disability Network of Mid-Michigan?

The Person-Centered Options Counselor (PCOC) is dedicated to promoting independence for individuals with disabilities by providing support and guidance as they explore long-term services and medical coverage through the State Health Insurance Partnership (SHIP) program. This position plays a crucial role in helping individuals navigate their personalized needs and preferences, ensuring access to the right resources for independent living.

Key Responsibilities:

  • Respond to referrals and service requests, offering timely, appropriate referrals to community resources.
  • Assist individuals in navigating long-term support services tailored to their needs and preferences.
  • Conduct comprehensive assessments to identify specific needs, desires, and goals of individuals.
  • Support clients in goal setting and developing action plans to achieve their independent living goals.
  • Provide unbiased counseling regarding healthcare coverage, subsidy eligibility, Medicare preventive services, and prescription drug plans.
  • Participate in outreach and educational initiatives to promote PCOC/SHIP services and build community connections.
  • Collaborate with the No Wrong Door (NWD) Program Manager to meet program objectives.
  • Maintain regular contact with community members through telephone, email, and in-person meetings.
  • Prepare written reports and billing as required by funding sources.
  • Ensure services are delivered in strict adherence to the DNMM Code of Ethics, legal, governmental, and contractual requirements, as well as the Independent Living Philosophy.
  • Maintain positive relationships with contracting agencies and community partners.
  • Keep accurate consumer data and financial records in compliance with relevant laws and funding requirements.
  • Attend DNMM Team and community meetings as assigned by the Team Leader.
  • Participate in initial and refresher training courses as required.
  • Promote independence and advocate for individuals with disabilities.
  • Maintain all client files and complete documentation accurately and timely.
  • Complete necessary paperwork such as travel logs, time sheets, and reports.

Skills & Qualifications:

  • Knowledge of community resources, including long-term support services.
  • Familiarity with HIPAA compliance and Mandated Reporter guidelines.
  • Knowledge of person-centered processes and experience with developmental and mental health disabilities.
  • Strong communication skills (oral and written) with excellent problem-solving and organizational abilities.
  • Ability to work independently, maintain consumer confidentiality, and promote consumer choice.
  • Ability to work in diverse environments, including low-income neighborhoods and non-accessible spaces.
  • High School Diploma or G.E.D. required; Bachelor’s degree in a health or human services-related field or equivalent life experience preferred.
  • Certifications such as AIRS Certification, Options Counseling Certification, MMAP Certified, SOAR Certified, Motivational Interviewing, and Trauma-Informed Care are highly preferred.

Key Tasks & Time Allocation:

  • Face-to-face contact: 50%
  • Paperwork/Documentation: 20%
  • Travel: 20%
  • Agency Meetings/Training: 10%

Working Conditions:

  • Travel to and from community locations within a 12-county area (Alcona, Arenac, Bat, Clare, Gladwin, Gratiot, Iosco, Isabella, Midland, Ogemaw, Roscommon, Saginaw), approximately 35-40% of the time.
  • Entering homes or community locations with potential obstacles such as steps and stairs.
  • The position is based on a full-time schedule (40 hours per week) with an 8-hour shift, Monday to Friday, with some flexibility required outside regular business hours.
