Job Requirements :
Education : High School Diploma or GED preferred.
Experience : 1 year experience in patient care in a home health, long term care, hospice and / or acute care setting.
Must be able to read, write, verbally report important information about patients.
Current BLS certification
Possess valid state driver’s license and reliable automobile and be willing to operate personal car necessitated by nature of job.
Proof of current automobile insurance with minimum state financial responsibility limits
Current health certificate / physical examination and TB testing.
Ability to lift up to 50 lbs.
Ability to operate a desktop and laptop computer.
Ability to stand and / or sit for long periods of time.
Ability to climb stairs and to provide services in a patient’s home environment.
Ability to lift, pull, push, and manipulate equipment and patients which requires strength, gross motor and fine motor coordination.
Your responsibilities will be to :
Provide patients with personal care directed by the RN and plan of care.
Assist patients in bathing, care of mouth, skin, and hair, and assist patients in and out of bed and with ambulation.
Perform turns of patients in bed and assist in positioning patients for comfort as well as changing bed linens as needed.
Report changes in patient's condition and needs.
Prioritize patient comfort, quality of life and safety.
Maintain up to date patient records and chart in a timely manner according to policy.
Schedule : Usual hours fall within the 7 : 45-5, Monday through Friday work week.
- Employee will be eligible for a $1000 bonus following the first of the month of their one-year anniversary. Additionally, employee must be considered in good standing with the organization and have a favorable employment record.
- Preferred applicant will be asked to pass a background check and drug screen following a job offer.
- New employees are required to complete tuberculosis screening and provide proof of hepatitis B vaccination or signed declination.
