Location : Puyallup, Washington📄 Job Type : Contract – 13 weeks (Possible Extension)🕗 Schedule : Monday, Tuesday, Thursday, and Friday | 07 : 00 AM – 05 : 00 PM💰 Pay : $45.00 – $55.00 / hour (Based on experience & credentials)Responsibilities : ✅ Coordinate Patient Care Plans – Develop, implement, and oversee individualized care plans, ensuring alignment with patients' medical needs and treatment goals✅ Serve as a Liaison – Act as the primary point of contact between patients, families, healthcare providers, and support staff to facilitate seamless communication and care transitions✅ Monitor Patient Progress – Assess patients' conditions, track treatment effectiveness, and collaborate with medical teams to adjust care plans✅ Educate Patients and Families – Provide guidance on medical conditions, treatment options, medications, and post-discharge care to promote health literacy and adherence✅ Ensure Compliance and Quality Standards – Adhere to healthcare regulations, institutional policies, and best practices to maintain high-quality patient care and safety✅ Facilitate Access to Resources – Assist patients in navigating healthcare services, including referrals, insurance coordination, and access to community support programsRequired Qualifications : 🎓 Graduation from an accredited school of nursing🩺 Active and unrestricted Washington State RN license💉 Basic Life Support (BLS) certification (AHA) or CPR / AED for the Professional Rescuer (American Red Cross)📋 Minimum 3 years of experience in Family Practice💻 EPIC knowledge and proficiencyPride Global offers eligible employee’s comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance, and employee discounts with preferred vendors.
