Job Type : Contract – 13 weeks (Possible Extension)
Schedule : Monday, Tuesday, Thursday, and Friday | 07 : 00 AM – 05 : 00 PM
Pay : $45.00 – $55.00 / hour (Based on experience & credentials)
Responsibilities :
Coordinate Patient Care Plans – Develop, implement, and oversee individualized care plans, ensuring alignment with patients' medical needs and treatment goals
Serve as a Liaison – Act as the primary point of contact between patients, families, healthcare providers, and support staff to facilitate seamless communication and care transitions
Monitor Patient Progress – Assess patients' conditions, track treatment effectiveness, and collaborate with medical teams to adjust care plans
Educate Patients and Families – Provide guidance on medical conditions, treatment options, medications, and post-discharge care to promote health literacy and adherence
Ensure Compliance and Quality Standards – Adhere to healthcare regulations, institutional policies, and best practices to maintain high-quality patient care and safety
Facilitate Access to Resources – Assist patients in navigating healthcare services, including referrals, insurance coordination, and access to community support programs
Required Qualifications :
Graduation from an accredited school of nursing
Active and unrestricted Washington State RN license
Basic Life Support (BLS) certification (AHA) or CPR / AED for the Professional Rescuer (American Red Cross)
Minimum 3 years of experience in Family Practice
EPIC knowledge and proficiency
